APPLICATION FORMS

Application forms are available from the Awards Administrator, who can be contacted by e-mail on [email protected], by telephone/fax on 020 7326 0995, or by post at Conservation Awards, 702 The Chandlery, 50 Westminster Bridge Road, London SE1 7QY.

PDF Forms
All the applications forms apart from the Digital Preservation Award form are available from this page to download as PDF files. To open, complete and print these you will need the free Acrobat Reader program from Adobe, which can be downloaded from the Adobe UK Web site at http://www.adobe.co.uk/products/acrobat/readstep.html.
Please note that the free Acrobat Reader software does not allow you to save a copy of the completed form. Although you can complete it using this software you must print it out in order to generate copies for submission and for your own reference.
